What is a LTC (long term care) pharmacy?

All patients are in a setting where a caregiver is administering or monitoring the drug regimens:

Nursing Homes, Assisted Living Center, Group Homes, Hospice Patients, Medical at Home Patients

98% of medications are delivered directly to caregiver

Caregiver keeps a record of drugs received and administered to the patient

* Typically the exceptions are hospice & stat orders, which are occasionally picked up at the pharmacy door due to urgency of medication administration

What makes this pharmacy different?

When medication is administered by a caregiver, there is increased need for accountability. Maybe the nurses switch shift, how does the next nurse know if a medication was administered? Special packaging is provided by the pharmacy and nurses record on the card and in chart to have a double check on administration records. When multiple patients are being cared for, the special packaging aids the caregiver in knowing how many doses are remaining and if a medication should be reordered.
